In-Home and Community Supports Provider with Crawford County Mental Health Awareness Program (CHAPS)
Seeking part-time In-Home and Community Supports Provider. Candidate must possess valid driver’s license, high school diploma or equivalent, and strong interpersonal and communication skills. Previous work experience in the Intellectual Disability and/or Human Services field preferred.
Duties to include:
- Connecting participants to community resources, including both physical and mental health professionals
- Assisting participants with independent living and social skills
- Other activities which positively impact progress made toward goals as outlined in participants’ Individualized Support Plans (ISP’s)
